Health Care for the Homeless Project provides Accessible, Quality Care for Individuals and Families who are Homeless through integrated Medical, Mental Health, Outreach and Social Services.
Mission Statement
The mission of the Health Care for the Homeless Project is to reach and care for those individuals most in need of help who are least likely to receive services in other settings. We provide access to professional and integrated medical, mental health, and social services. Our committment is to address both immediate needs and reintegration of persons into the larger community.
We achieve our mission through cost-effective direct services, prevention, advocacy, research, and by enhancing the education of professional care providers, policy makers, and the general public. Our approach to health care is team-oriented, client-driven, respectful, creative, kind, and non-judgmental.
Health Care for the Homeless Project Clinics
Sumner Street Clinic
Located at the Institute for Human Services Men’s Homeless Shelter in Iwiliei. Psychiatric care, actute medical care, TB screening, follow-up and referral services are avaialble on a first come, first served basis. Case Management services include: assistance with benefit attainment, money management, housing, food, and clothing. Psychiatric servcies available, walk-in during clinic hours, no appointments are given. Clinic hours are 7:am - 12pm Monday through Friday.

OHANA Project (Oahu Health Access and Network Association)
The OHANA Project provides in-reach to Queen’s Medical Center in Downtown Honolulu. OHANA works with individuals who are in the hospital and will be released with no housing. OHANA provides follow-up care after a person is discharged from the hospital. Case management, medical servcies, referrals are all provided at the OHANA clinic located in Kalihi Valley and the Kohou Clinic.

Ka’a'ahi Street Clinic
Located at the Institute for Human Servcies Women and Family Homeless Shelter, the Ka’a'ahi Street Clinic provides an array of services including: psychiatric care, first aid, acute medical care, TB screening, infection control, case managment, follow-up and referral servcies.

Kohou Street Clinic
The Kohou Street Clinic is the home base for the PATH outreach workers, along with HPHA funded outreach workers, the Community Based Case Managment Team (CBCM) and the Administrative Staff of Health Care for the Homeless Project. Monday, Tuesday, Thursday and Friday, clients can go to the Kohou Clinic for Case Management services, psychiatric services and representative payee.
