What To Bring

To get WIC you need to bring.

1. Proof of income such as pay stubs, tax return or medical benefit papers.
2. Proof of identity such as a state ID card or birth certificate for you and each child.
3. Proof of Hawaii address such as a utility bill or rent receipt.
4. All children you want to receive WIC.
If you are pregnant. Please bring a note from your doctor that certifies you are pregnant.

If you just gave birth. Please bring your baby and papers from the hospital about you.

We can help you with breastfeeding!

What will happen at the appointment.

* WIC staff will certify you for WIC!
* We check your height, weight, diet and do a simple finger prick blood test if needed.
* You will also learn now the WIC program works including learning about how to buy WIC food.
* We will do our best to answer questions about food and health and give you referrals for additional services.
