We provide comprehensive healthcare for our patients.
A Patient-Centered Medical Home (PCMH) is not a building, house, or hospital, but rather an approach to providing comprehensive healthcare to our patients and their families.
A Medical Home is called a “home” because we want this clinic to be the first place you think of for your medical needs. Our goal is to make it easy and comfortable to get the care you need in a way that works best for you.
At Kalihi-Palama Health Center (KPHC), you can choose your personal primary care provider (PCP). We provide an innovative, team-based approach to providing healthcare services. A partnership is developed between you, your PCP, and our healthcare team. Together following evidence-based guidelines for medical care; our team will coordinate to provide the best healthcare services possible for you.
